Community Care
for the Elderly
Independent Living Systems (ILS) is one of the three Lead Agencies for Miami-Dade County, as well as the Lead Agency for Monroe County.
As a Lead Agency, ILS receives Community Care for the Elderly (CCE) and Home Care for the Elderly (HCE) funding to implement service care plans to adults aged 60 and over that provide in-home support.
Our team of Case Managers work with the members and their loved ones to ensure needs are being met and members remain as independent as possible in the safety and comfort of their homes to prevent or delay institutionalization.

The HCE program is available to members who have an eligible caregiver that provides ongoing support in their homes. If a member and their caregivers meet the eligibility criteria, caregivers are provided with a monthly stipend of $160 to provide support for the member’s care.
As a Lead Agency client, members have the benefit of working one-on-one with their assigned Case Manager to assist with any needed resources and referrals, as well as advocating for them as needed.
ILS is a Hospital 2 Home Provider for Miami-Dade County with funding received from the American Rescue Plan (ARP). Working closely with the local Area Agency on Aging, this program started as a pilot in 2020 during the COVID-19 Pandemic as a program to help adults 60 and over with reducing hospitalizations after a hospital discharge. By providing in-home services of personal care, homemaker and home-delivered meals, the program’s outcome statistics have demonstrated that in-home support for up to 90 days after a discharge drastically reduce hospital readmittance rates.
ILS is in the process of expanding this program to Monroe County by late 2022.

ILS is also an Emergency Home Energy Assistance Program (EHEAP) Provider for Miami-Dade and Monroe County. Our Case Management staff is fully trained to complete EHEAP applications for adults sixty year and older with a light bill that is past due or have a final notice. Applications are completed in person by appointments only.
