CalAIM Resources
At Independent Living Systems (ILS), we know that complex health and social care needs extend beyond physical well-being. Your needs may span across housing, transportation, healthy food and so much more. ILS supports these needs through CalAIM’s Enhanced Care Management program and Community Supports services.
What is CalAIM?
CalAIM stands for California Advancing and Innovating Medi-Cal. The program hopes to improve health outcomes for people with Medi-Cal (California’s Medicaid insurance).
ILS is here to help you access services, so you can live a well-rounded life. We partner with health plans to support two of CalAIM’s programs — Enhanced Care Management and Community Supports services.
Enhanced Care Management Program
The Enhanced Care Management program is for eligible Medi-Cal members. The program guides you with your health care needs and related community services. When you enroll, you get a dedicated care manager at no cost.
Your dedicated care manager helps you:
- Apply to free or low-cost community services
- Coordinate rides to doctors’ appointments
- Find doctors and specialists
- Get support for housing, nutritious food, and medical equipment
- Receive and manage medications
- Schedule health appointments and follow-up care
Community Supports Services
Community Supports services are also eligible for Medi-Cal members. These services can help you with housing, food, and other needs. We focus on people and families whose health issues result from unstable housing. When you enroll, you get a housing navigator at no cost. Those services include:
Housing Navigation and Tenancy Sustaining Services
Sobering Centers
Recuperative Care (Medical Respite)
Day Habilitation Programs
Personal Care and Homemaker Services
Respite Care
Medically Tailored Meals
Community Transition Services/Nursing Facility Transition to a Lower Level of Care
Environmental Accessibility Adaptations (Home Modifications)
Your dedicated housing navigator helps you:
Find housing
Housing Transition Navigation Services support you during the rental process.
Pay for housing
Housing Deposits fund one-time services that help you set up a household.
Remain in secure housing
Housing Tenancy and Sustaining Services focus on life skills, such as budgeting, and referrals for community resources.
Who qualifies?
You or your family may qualify for the Enhanced Care Management program and Community Support services if:
- You have a serious mental health condition or a hard time with substance use.
- You are living in a skilled nursing facility but would rather live in your community.
- You are without housing.
- You were released from prison.
- You are an eligible Medi-Cal member with one of ILS’ health plan partners.
- You have complex health issues and have been to the hospital or emergency department five or more times in a six-month period.
