History

Founded in 2001, Independent Living Systems (ILS) was formed to assist health plans and other organizations in the delivery of community based services as an alternative to costly institutional based care.

In developing and implementing one of the country’s first long-term care diversion program in the country, we learned that nearly 70% of people residing in nursing homes would not need to be there if they had access to community based resources to support them. With an average cost of a year-long stay in a nursing home at $60,000, ILS developed the Managed Long-Term Services and Supports Program to reduce those costs to an average of $25,000. Improving costs and quality, combined with a patient’s preference to live at home has been the driving force behind the development of our future services.

In 2005, we added Meals and Nutritional Counseling to provide targeted nutrition benefits. We employ registered dietitians who assess nutritional needs and produce customized menus for home-delivered meals.

In 2010, we introduced Transitional Care Management to reduce readmissions of patients back into the hospital. Eighteen percent of admitted Medicare patients are readmitted to the hospital within 30 days for the same condition. ILS’ model of care has successfully reduced readmissions by as much as 50 percent. That same year, Care Management was introduced for the care management of Medicare and Medicaid populations. Care Management begins with stratifying a population based on acuity; a comprehensive assessment is performed on the member’s health, social, and environmental needs; and an individualized plan is produced documenting a member’s problems, goals, and interventions, serving as a roadmap to recovery.

In 2013, ILS completed the acquisition of a business and technology provider that delivers fully integrated Third Party Administration solutions.

As healthcare delivery continues to gravitate toward financial realignment initiatives such as in the Duals Demonstration programs being implemented across the country, ILS is poised to be a driving force in the transition of healthcare back to the home and community.
